When to use this template

Request a ban reduction, ban removal, or blacklist reinstatement review.

How to use it

Use the designated private appeal process. Disciplinary appeals should be submitted within 30 days of the decision. Reinstatement review is normally available after 30 days unless the decision notice sets different terms. Submitting does not guarantee approval.

Privacy and handling

Submit completed copies only through the clan’s appropriate private ticket or authorized staff channel. Do not publish completed applications, appeals, leave requests, or incident records. Share only the information necessary for the request; never include passwords or private account access.
